9/29/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Josiah was observed in the session on task and displaying excellent pro social behaviors (e.g. helping counselor during family activity, sharing his thoughts when needed, and implementing relaxation strategies). Josiah was focused and on task. Josiah was having appropriate conversation with everyone in the home. Josiah was respectful to mom and QMHP-C. Josiah displayed good manners, saying please and excuse me when trying to get someone’s attention or asking for help. Behavior: Josiah presented cooperative and normal behaviors as he did not need any redirection from QMHP-C as well as him answering open -ended questions and being praised by QMHP-C. At times within the session, he began hostile evident by him screaming the lady was a liar while placing the blame on the next-door neighbor. Intervention: QMHP-C reminded Josiah of ISP objectives. QMHP-C explored the positive behavior options he was exhibiting with Josiah to support his willingness to continue to make good decisions and have positive behaviors in the home. QMHP-C talked with Josiah about the importance of adhering to rules and expectations in school, at home and in the community. QMHP-C held consultations with Josiah and mom about effective intervention strategies that build on Josiah's strengths and compensate for his negative behaviors. Intervention: QMHP-C encouraged Josiah’s mom to provide frequent positive reinforcement to maintain Josiah's interest and motivation in maintaining positive behaviors and completing his school work and chores. QMHP-C implemented positive reward system regarding Josiah behaviors and choices. QMHP-C observed Josiah complying with rules and expectations Response: Josiah displayed positive interaction with his family and was supported with building healthy relationships. Josiah presents a positive attitude when given the opportunity to earn a reward. Josiah thanked QMHP-C for the incentive. Josiah was able to develop a list of resourceful people who he could turn to for support, assistance or instruction. Josiah understood to seek support at least once from parent, teacher, or counselor during the week. Response: Mother agreed to record three to five responsible behaviors by Josiah during the week that pertain to his behavior. Mother agreed to start focusing and acknowledging Josiah’s positive behaviors instead of his negative behaviors. Josiah was supported for using positive techniques for obtaining attention and confirmation of caring. Josiah responded in a positive manner acknowledging the positive reinforcement that QMHP-C provided by saying “thank you” while presenting a smile. Josiah utilized relaxation strategies promptly.
